Statewide Leadership

By June 30, 2013:

NCCHW Staff will serve on 19 Boards, Councils, Task Forces and Steering Committees that impact action and success in the three areas of focus--healthy aging, worksite wellness, and healthy weight for children, youth and young adults.

Healthy Children 0-18

By June 30, 2013: 

An additional 22,236 North Carolina children and youth being served in schools and childcare centers will adopt at least one policy or environmental change that supports moderate physical activity and/or healthy eating. This will bring the total number of children and youth benefiting from such policies to 79,685. 

The number of children served in childcare and in schools that support policies and create environments which support healthy eating and physical activity. Promote Environments that Provide Access to Healthy Food and Physical Activity Among Children and Youth Ages 0-18
Healthy Older Adults

By June 30, 2013:

Teams from four additional counties complete EMMA training and initiate at least one health promoting/supporting policy at senior meal programs/sites. This will bring the total number of participating counties to nine. 

 The number of community teams completing EMMA training and receiving technical assistance from the NCCHW that initiate at least one health promoting/supporting policy change at senior meal program/sites.  Promote Behaviors that Extend a Healthy Lifespan Among North Carolinians Approaching Later Adulthood
Healthy Older Adults

By June 30, 2013:

At least 130 seniors from three additional counties completing EMMA training will benefit from the initiation of at least one health promoting/supporting policy. This will bring the total number of seniors benefiting from project EMMA to a total of 400. 

The number of older adults served in health promoting/supporting senior meal programs that provide access to healthy food, increased physical activity and tobacco free policies. Promote Behaviors that Extend a Healthy Lifespan Among North Carolinians Approaching Later Adulthood
Healthy Workforce

By June 30, 2013:

107 additional worksites implement at least one health promoting/supporting policy change. This will bring the total number of worksites reached to 133.

The number of worksites completing WorkHealthy America training and receiving technical assistance from the NCCHW that initiate at least one health promoting or supporting policy change.

Promote Best Practices That Support Healthful Living for Employees
Healthy Workforce

By June 30, 2013

3,927 employees from additional worksites will benefit from implementation of at least one health promoting/supporting policy change. This will bring the total number of employees benefiting from policy changes to 5,607.

The number of employees working in health promoting/supporting environments as a result of those policy changes.

Health Equity

By June 30, 2013:

At least one initiative/project/activity from each of the NCCHW areas of focus will include a Tier 1 county.

The number of initiatives/projects/activities of the NCCHW that reach Tier 1 counties. The N.C. Department of Commerce annually rates all 100 N.C. counties based on economic well-being and assigns a Tier designation. The 40 most distressed counties are designated Tier 1. Factors in determining rank include poverty rates, median household incomes and assessed property values among others factors.  Include Promotion of Health Equity as an Emphasis in the Work of the NCCHW
